Five Core Services
|
CDR's Outreach Program offers five core services and provides an opportunity for individuals to empower themselves, live in the
community and maintain their own independent living.

Information and Referral:
We provide you with information about individual rights, community services, equipment, accessibility, and much more.
If we do not have the information or specific service you need, we try to locate it.
|

Independent Living Skills Training:
We can provide you with support, instruction and assistance to help you learn the skills you need to be independent.
CDR can assist you in learning to budget, travel on a bus, manage personal care attendants, or advocate for yourself.

Peer Support Services:
Outreach workers and advocates, who themselves have disabilities, provide peer support and serve as role models.
|
| |
Advocacy:
Our staff assist people in getting the benefits and rights they are entitled to. We teach people to be self advocates who
know their rights. We advocate to help people get culturally and linguistically appropriate services. Our systems
advocacy changes local, state and federal policies which affect the lives of individuals with disabilities.

Community Reintegration:
CDR is a leader in helping get people out of nursing homes. We help people make the transition back into their own
homes by coordinating services and advocating for their rights.
