Case Management
Case management helps people with disabilities to live and participate in the community according to their personal goals, preferences and needs. A primary responsibility of the case manager is to empower consumers to make informed choices about their lives and futures.
Case management services include:
1. Gathering information and developing personal goals
2. Developing the Individualized Service Plan (ISP)
- Acting as a liaison or broker between the consumer and service providers
- Promoting self advocacy and encouraging the consumer to put his plan into action to the greatest extent possible
- Ensuring that the process of following the ISP does not compromise the participant's goals, preferences and needs
3. Overseeing the Individualized Service Environment
- ensuring that the ISP is effective and helps the consumer to live in the community
- adapting the ISP, when necessary, as the consumer's needs change, accomplishments occur, or temporary setbacks happen
- maintaining ongoing contact with the consumer and all service providers
- advocating or intervening with service providers, when necessary, to ensure that services are provided in a manner that meets the individual's needs and preferences. Our case managers strive to integrate our participants into existing community-based services and are available 24-hours every day by pager to handle emergencies
